How to create a referral

Step 1:

GP to log in to the HAS Admin https://book.health.gov.sg/admin with Singpass.

Step 2:

Click on "Create New Referral" in the dashboard.

Step 3:

Enter patient’s NRIC; name and contact will auto-populate if available.

Otherwise, fill in the patient's details.

Step 4:

Select the referral destination. Ensure that the correct payment class (Subsidised / Private) is selected before proceeding.

Step 5:

Submit the referral via the FormSG link.

• Subsidised referrals will be routed to the CHAS form.
• Private referrals will be routed to the referral notes form.
• Note: Appointment dates are coordinated by call centre coordinators.
