Important Notes: Referrals will be stalled and cannot proceed unless new SingHealth patients details have been submitted. Submitting patient details via FormSG for SingHealth referrals is a one-time process per new SingHealth patient only. It will not need to be repeated for subsequent referrals for the same patient to SingHealth SOCs.

Patients that have never been registered with SingHealth cluster institutions need to undergo a one-time registration process. This allows the SingHealth SOC to ensure patients are enrolled with all necessary information before they can be processed for appointments. Hence, they are required to submit their particulars before a referral for them can be processed.
There are 2 routes for submission as listed below -
By the Patient - Patient’s are sent a FormSG link via SMS to fill out their particulars and submit. They are sent this SMS as part of your referrals request submission. Only once the form is completed and submitted, can the referrals be processed. Please remind your patient to submit this form as soon as possible so their referral appointment can be processed further.
By the Doctor - You would be able to submit this form on behalf of the patient by tapping on the “Submit on behalf of patient” button located in the referral listing section of your HAS Referrals page.
